Flu Shots for Students and Staff

Daniel Pharmacy will be giving flu shots at St. Edmond on Oct. 7th to students and staff (including coaches). If you want your student to get the shot please fill out the appropriate attached form and attach a copy of the front and back of your insurance card. These forms need to be turned in to the nurses office by Monday Oct. 5th.
